Background: Gallstone ileus post-cholecystectomy is extremely rare surgical emergency causing mechanical bowel obstruction. This condition usually misdiagnosed due to its non-specific clinical features. Radiological investigations are essential evaluate and diagnose gallstone cases. Case Presentation: A 92-year-old female patient, cholecystomized, presented our hospital with a one-week history of an acute confusional state, generalized abdominal pain, vomiting. The physical examination showed disoriented patient distended abdomen tenderness, especially in the epigastric area. Hyperactive sounds were audible. Laboratory non-specific. plain X-ray was insignificant. ultrasound marked gaseous distention. Then, CT scan performed, revealing impression small obstruction no evidence fistula formation. So, intervention decided, 8-gram exteriorized through enterolithotomy. postoperative period uneventful. Discussion: unusual entity that often affects elderly females. features include distension, nausea, Diagnosis relies on imaging examination. Abdominal contrast provides definitive diagnostic tool. options manage can be either one or two-staged Conclusion: Due etiologic origin cases, we report case state found caused by detected ileus, lacking
